Regular and Follow-up Status Call Questionnaire – Example

Description

Regular and follow-up status calls are conducted by staff and volunteers who contact mHealth program participants to identify any problems or technical support needs. Calls are made one week after the initial mHealth program training with follow-up calls monthly thereafter. Volunteers can contact between 10 to 50 participants per week. Each call lasts 3 to 5 minutes and is recorded on a brief questionnaire form. Status calls help to maintain participant engagement and adoption.
