Program Discharge GuideDescriptionEnrollment in a remote monitoring program is typically for a fixed period. Services are usually terminated when a patient completes the recommended care and/ or meets the goals of the program. Services may also be terminated because patients no longer qualify to receive services under the standard policies and guidelines that are in place for participation in the program.
