Description

A medication discrepancy tool (MDT) that was used in a technology-enhanced Care Transitions Intervention® (CTI) program. The MDT is designed to facilitate reconciliation of a patient’s medication regimen across settings and prescribers.

Audience

Any health service provider or allied professional that ensures appropriate care, self-management support, and follow-up for patients transitioning from hospital to home or between other care settings.
