Clinical Processes: A Guide from the Care Transitions Technology ToolkitDescriptionIn order to standardize the delivery of technology-enhanced transitional care, guidelines or protocols outlining key clinical processes should be developed. Clinical care processes to address include patient assessment and enrollment, patient care plans during care transitions, patient education, and how clinicians should respond to data or alerts generated by related technologies (e.g., remote patient monitoring technologies).
